The Patient Who Cannot Name What They Feel: Alexithymia and Treatment Response Across Common Mental Disorders

Abstract
Alexithymia is frequently observed in depression, anxiety-related conditions, trauma-related disorders, and eating pathology, but its value for predicting treatment response remains uncertain. This research uses critical integrative review. The literature did not support alexithymia as a fixed marker of treatment failure. Difficulty identifying feelings appeared more clinically relevant than a high total score, and several associations weakened after baseline depression or general distress was considered. Treatment studies nevertheless suggested that alexithymia could matter when therapy required emotional discrimination, experiential engagement, or verbal use of affect before those capacities were sufficiently available. The synthesis developed emotional access mismatch as a conditional analytical proposition describing a treatment-patient interaction rather than a diagnostic category. Its influence should be weaker when treatment does not initially depend on emotional articulation and should diminish when therapy successfully scaffolds emotional access. Alexithymia may therefore inform formulation and pacing without becoming a reason to exclude patients from evidence-based psychotherapy.
